Gatifloxacin-Induced Hyperglycemia.

Abstract:

Objective: To report a case of hyperglycemia in a patient receiving gatifloxacin for pneumonia.

Case Summary: A 70-year-old male with an extensive medical history, most notably restrictive lung disease requiring home oxygen, obstructive sleep apnea on continuous positive airway pressure, ischemic cardiomyopathy, and diabetes mellitus, was admitted for pneumonia and prescribed intravenous gatifloxacin (400 mg/d). After 3 days of therapy, the patient's blood glucose readings continued to increase and remain elevated despite clinical improvement from his pneumonia and resolved after discontinuation of the drug.

Discussion: Gatifloxacin is associated with disturbances in blood glucose, with some severe cases of hypoglycemia and hyperglycemia occurring in patients with diabetes and patients not previously diagnosed with diabetes usually within 3 days of treatment. Our case confirms this finding and offers possible explanations for such reporting.

Conclusions: Time of administration of gatifloxacin and subsequent hyperglycemia in the absence of other causes suggest that gatifloxacin is the likely cause. Clinicians should be aware of this complication in patients taking gatifloxacin with associated hyperglycemia.
